Waivers

All participants at Orlando Aerial Arts are required to fill out a registration form, COVID Questionnaire, and an age-appropriate participation waiver before starting class.

All students, regardless of age, must complete the COVID Questionnaire. Then either email a copy to [email protected] or bring in a paper copy to your first class.

Complete your waivers

Completely fill out all three (3) documents: the registration form, the COVID questionnaire, and the age waiver. Then either email a copy to [email protected] or bring in a paper copy to your first class.

Then either email a copy to [email protected] or bring in a paper copy of your waivers with you on your first visit to Orlando Aerial Arts! Wear comfortable clothing that allows a full range of movement.
